    In making this application the undersigned agrees to the rules and regulations of the New Windsor Telephone Company as set forth in the exchange tariff, and to any general changes in the rules, regulations, tariffs or rates for the service furnished under this application. This application becomes a contract when accepted in writing by the New Windsor Telephone Company.

    OPTIONAL BATTERY BACKUP POWER FOR VOICE

    Battery Backup Power For Residential Voice Telephone Services During Power Outages

    For many years, your home telephone would allow you to stay connected to emergency voice services during a power outage. However, if your residential voice telephone service is provided using fiber optics rather than the traditional copper-based line, the service requires backup battery power to continue functioning during an outage. To avoid a disruption of home voice service during an outage - and to maintain the ability to connect to 911 emergency services — New Windsor Communications gives you the option to add an Uninterruptible Power Supply (UPS) Battery back-up for an additional monthly fee.

    What Your Battery Can - and Can't - Do for You

    New Windsor Communications backup battery options for telephone allows you to continue to use your home voice services during a power outage. Without a backup battery or alternate backup source such as a generator, you would not be able to make any calls, including emergency calls to 911. The only way to maintain the ability to use your telephone is by using some form of backup power.

    Our backup battery options do not provide power to any services other than voice. Cordless telephones, home security systems, medical monitoring devices and other equipment will not run on a residential telephone backup battery.

    Purchase and Battery Replacement Options

    If you are concerned about being able to contact 911 emergency services during a power outage, a backup battery may be a good option for you. New Windsor Communications offers an 8-hour backup battery. The 8-hour battery is approximately 2 1/2 pounds and roughly 4" x 2" x 7" and you can lease for $3.00/month. New Windsor Communications will provide and install at no cost to you, a replacement battery, upon appointment, when it becomes known that the battery needs to be replaced. It is your responsibility to notify us when the leased battery needs replaced.

    Expected Backup Power Duration

    Backup batteries are typically expected to last their noted duration in standby mode (8hr) without commercial power.

    Instructions for Proper Care, Replacement and Use of Your Battery

    Improper storage and environmental factors such as temperature can shorten the useful life of the battery, so it is recommended to store the battery above 320 F and below 104 0 F. The battery is rechargeable but will not last forever. If an indicator light changes color or possibly your device starts beeping, it could be time to replace.     High Speed Internet

    All prices shown are monthly with the exception of installation charge. Prices do not include regulatory fees, surcharges and/or taxes. Gigabit is maximum capacity. Speed is likely to be slower because transport overhead and other locations on the Internet are not yet gigabit-capable. Very few wireless routers support gigabit connection speeds. Gigabit speeds are “best-effort” and not guaranteed. Speeds in the 700 to 900 Mbps range are most common on Gigabit tier.

    NEW WINDSOR CABLE TV, INC. - HIGH SPEED INTERNET TERMS AND CONDITIONS

    1) By signing this registration form and agreement, you (customer) must be at least 18 years of age and agree to the following terms and conditions of the service described below.

    2) These services are provided by New Windsor Cable TV, Inc., PO Box 488, New Windsor, IL 61465. This agreement shall be governed by and construed in accordance with the laws of the state of Illinois applicable to contracts to be performed entirely within the State.

    3) The ONT (Optic Network Terminal) will be provided by New Windsor Cable TV, Inc and, in consideration for New Windsor Cable TV, Inc providing the ONT customer agrees to pay the installation fee and the monthly service payment. Both New Windsor Cable TV, Inc and Customer may terminate this Agreement at any time. Such notice is effective when given and Customer is obligated to pay for service through the termination date. All equipment must be returned at time of termination.

    4) In order to properly maintain the New Windsor Cable TV, Inc High Speed Internet network, New Windsor Cable TV, Inc reserves the right to change Customer’s IP, gateway and subnet addresses.

    5) All High-Speed Internet charges will be billed by New Windsor Cable TV, Inc at the first of each month, included in your New Windsor Telephone Statement, payable by the 21st of that same month. Failure to pay by the specified due date may result in your service being discontinued. A reconnect fee will be required to reestablish service. A deposit may also be required.

    6) New Windsor Cable TV, Inc makes no express or implied warranty of any kind. There is no express or implied warranty of merchantability or fitness for a particular purpose for the services to be provided by New Windsor Cable TV, Inc. New Windsor Cable TV, Inc makes no attempt to verify accurate receipt of any message and is not responsible for any loss of data resulting from delays, nondeliveries, incorrect deliveries or service interruptions, including those caused by the negligence, errors or omissions of New Windsor Cable TV, Inc, nor for consequential damages regardless of their cause.

    7) Customer agrees to comply with the Terms governing use of the New Windsor Cable TV, Inc network as set forth in the New Windsor Cable TV, Inc’ “Acceptable Use Policy”, as they may change from time to time. This may be found at www.nwtelephone.com.

    8) By accessing and using the New Windsor Cable TV, Inc user is consenting to system monitoring for law enforcement and other purposes. Unauthorized use of, or access to, this computer system may subject users to criminal prosecution and penalties. User agrees to indemnify New Windsor Cable TV, Inc. against liability for any and all use of user’s account.
